To get started in PT, have your physician complete a prescription and fax it to (817) 727-4319 or email newpatient@premierrehab.org. We take it from there.
How to refer
Fax the physical therapy prescription to (817) 727-4319. Our scheduling team contacts the patient to book at the clinic closest to them.
Send it to newpatient@premierrehab.org. Same-day follow-up with the patient.

The form
Some insurance companies require the physician to complete a physician prescription. Download it, have your physician complete it, and fax it to the location where you have your appointment.
